It would seem to me that you (and your doctor) have two options at this point. Increase the dose of Tramadol ER to 200mg or switch to something like methadone where the starting dose would be 5 or 10 mg.

I don't think that the issue with timing is so much about adsorption as it has to do with timing the drowsiness effect of the medication with your bedtime. RLS onset in childhood and teenage years happens, but it's less common than onset in a later age (AFAIK). If it's a genetic issue with iron transport, why is RLS in children rare?
