Thanks Polar Bear for responding. The afternoon symptoms can come on anytime from 2 pm to 4:30pm. I don't like taking an opioid too early or I may need another before I take my evening dose about 9 pm. I think I will try taking an Ultram today at 3 pm unless the symptoms start earlier. It is nice to know that, although I had delayed orgasm issues before I went on dopamine agonists, that my increasingly difficult time with orgasms is also a medication issue. I am in the process of shifting from Mirapex to Horizant (gabapentin encarbel) so perhaps it will get better.

I am in the middle of getting off 1 mg of Mirapex and going to Horizant (gabapentin encarbil). I tried to get my sleep doctor to prescribe me an opioid but he would only agree to Ultram (tramadol).I was able to cut the 1 mg in half to 0.5 mg of Mirapex using 500 mg of Ultram without too much pain. For me the problem with delayed orgasm started several years before I was diagnosed with RLS so it was not caused by medication. And that was about 25 years ago. Thanks Steve. I think your ratio of Horizant to Gabapentin sounds right. Didn't recall reading about big meals and lost of fluids reducing gabapentin absorption. I'll keep this in mind.

I have been on Horizant (gabapentin encarbil) for two nights. So far I don't find it more effective than taking gabapentin. I am getting off MIrapex due to augmentation and am shifting to gabapentin. I am using 50 mg of Tramadol every night along with gabapentin and declining doses of Mirapex. I had no idea that going from Mirapex to Requip was difficult. Never tried it. How about alternating Neurontin and Horizant. I am wondering what to do on nights where I won't get home until later in the evening and I don't have any Horizant with me. Could I take Neurontin on such a night without any...
