Alzheimers drugs...

Anything on your mind that isn't about RLS? It's nice to realize that there is life beyond this disease and have an opportunity to get to know our online family in a different context.

Alzheimers drugs...

Post by Frunobulax »

Continuing a discussion from viewtopic.php?p=108389#p108389, but it's off topic there...

This whole discussion reminds me very much of Alzheimers research.
The latest drug that is proving effective actually targets amyloid. I have been following the 'it is amyloid, the research was flawed on amyloid, now a drugh works' story for a while and I really don't know what to make of it. More data is needed.

"Lecanemab finds and removes amyloid that builds up and forms clumps in the brains of people with Alzheimer's disease. Lecanemab is the first drug to help improve the symptoms of Alzheimer's by slowing the disease."
We'll see. I can't access the full version of the article. A bit of google found very concerning reports that two participants may have died unexpectedly from the treatment (https://www.science.org/content/article ... -s-disease), and this nugget: "many of the people who received lecanemab in the phase 3 trial experienced amyloid-related imaging abnormalities (ARIA)—a term that describes brain swelling and bleeding. ARIA occurred in more than 21% of those who were on the drug; 17% experienced brain bleeds" . Also the Lancet questions if the observed small improvement in cognitive function is clinically relevant (https://www.thelancet.com/journals/lanc ... 1/fulltext).

The first question is always if the drug affects all-cause mortality in any way. What good is an Alzheimer drug that dramatically lowers the life expectancy? And for this question, a group of relatively young participants with very mild Alzheimers is not a good test group. (Ignoring all-cause mortality is unfortunately a widely used tactic -- thousands of cholesterol lowering trials show a reduction in cardiovascular events, but only very few look at all-cause mortality, and it turns out that lowering cholesterol does not change mortality, or even impacts it negatively -- except for very special groups, patients that had a cvd event in the last 6 months. See for example https://www.amjmed.com/article/S0002-93 ... 2/fulltext.)

Yikes!!

As for statins, my doctor tried to put me on them and I gave a firm 'no'.
