Welcome to

sundownerpilot, who suffers from severe RLS. Requip has helped and the dose has been titrated to the max. of 4mg daily. sundownerpilot badly needs additional recommendations.

The first thing will be to get you off of Requip, which can be challenging. Would your doctor be willing to prescribe an opioid to help you through the transition? If not, then you face a very challenging week with very little sleep. Once you are off, the normal process would be to have you try taking either gabapentin, Horizant or Lyrica. This helps many, but not everyone. If that doesn't work, then your only other option will be an opioid. Often this means a small dose of methadone, but any of the other opioids can be just as effective.
