Why Do Opioids Work?

I’ve been researching this quite heavily, but can’t come to any definitive answers. Hoping some other informed people can chime in. I know it’s not 100% understood, but there have to be some theories.

That is one of the ongoing research questions. I believe that the leading theory is that they interact with the dopamine and glutamate receptors that are paired with opioid receptors, much like how when the adenosine receptors lock up when iron levels are low and prevent the paired the A and G receptors from functioning. The same researcher at NIH who identified the role of adenosine as the link between iron and the A and G receptors is supposed to be working on this. But he had prior experience with adenosine, so his opioid work may not be going as quickly as he hoped.
